Jessa Nelson began developing a health care philosophy as a supervisor providing care for individuals with developmental disabilities. After purchasing a hobby farm in New Richmond, Jessa took on the director role in a senior care facility in New Richmond. During the next two years, she realized her philosophy of care went far beyond the very basic expectation of her employer. She felt strongly her responsibility to her clients went far beyond completing a schedule and making appointments. She began to accompany clients to appointments, educating herself on the very specific health issues and medical needs of senior clientele. In doing so, she became a strong medical advocate, encouraging doctors to reevaluate medications and common procedures used in senior clients and look at the whole client, not just a specific health issue. The result for many clients was improved health, better communication with care providers, a reduction in medication, and financial savings. In addition, she observed leaders among the staff learning about medications, improving their documentation, and being more assertive and thorough with client cares. These results were excellent, but Jessa had other concerns as well.

She was concerned about fees for care. She noticed some providers raising care levels and reassessing care points resulting in increasing fees to clients, with no change in staffing. The two didn't correlate and she saw families and clients struggle with the insecurity these countless financial fluctuations caused. It saddened her to see clients forego necessary care to avoid cost increases. After two years as director, Jessa determined a change was needed and decided to begin her own home. In 2005, Jessa and her husband, Paul, with the support of her friend and work partner, Kay Lewis, began looking for a location to build a home in which they could offer comprehensive, attentive care to senior in a small, comfortable, and permanent setting. With Paul's experience in making homes accessible and Jessa and Kay's care experience, they developed criteria for a home that created a homey atmosphere, accessible living, and ease of care provision. It was essential each room would have a private bath with shower, non-slip floor, superior lighting, seamless transitions, fans, and occupancy sensors.

A site was difficult to find, but in 2006 they purchased a beautiful house on 7 wooded acres bordering the New Richmond Golf Course. They broke ground in September 2006, working to maintain the architectural integrity of the unique house, known as the Swemby house, yet develop a functional home for providing care to seniors. The result is a comfortable, 7 bedroom, 9 bathroom home just ½ mile from Westfield's hospital in the City of New Richmond.
